CPC offers a variety of residential options throughout Monmouth County. The scope and purpose of the program is to prevent long term State/community inpatient care and increase independence, community integration and quality of life.
We believe in a least restrictive setting, “housing first” philosophy. CPC is focused on increasing our client’s independence and movement toward their recovery through support, life coaching, skill building, crisis intervention and linkages to community resources.
CPC’s supported residential services are provided to individuals living in shared apartments, group homes and individual apartments. All residents are 18 years or older and are responsible for self-supervision and compliance with their medications (with minimal assistance), maintaining the living space, shopping, cooking, cleaning and doing laundry. CPC staff provide individualized support with any issues in these areas.
Referrals for this program can be made directly through the Supported Housing Program. Referrals can also be made for consumers by community case managers, discharge planners or family members.
The Supported Housing Program is located in CPC’s offices at 6 Industrial Way West, Suite D-7.
